Penggunaan diuretik thiazide biasanya dikaitkan dengan kehilangan elektrolit, terutama kalium, tetapi juga natrium, klorida, bikarbonat, dan magnesium. Hilangnya elektrolit lain seperti fosfat, bromida dan iodida biasanya sedikit. Kalium dan magnesium penipisan dapat menyebabkan aritmia jantung dan henti jantung. Elektrolit lainnya yang berhubungan dengan komplikasi termasuk alkalosis metabolik dan hiponatremia, yang jarang mengancam kehidupan. Terapi dengan diuretik thiazide harus diberikan hati-hati pada pasien dengan atau yang cenderung untuk deplesi cairan dan elektrolit, termasuk pasien dengan aldosteronisme primer atau sekunder (mungkin memiliki kadar kalium yang rendah); orang-orang dengan berat atau berkepanjangan diare atau muntah-muntah; dan orang-orang dengan status gizi buruk.
