Paling neuroleptik agen memiliki aktivitas antikolinergik, yang tua pasien sangat sensitif. Clozapine dan rendah potensi agen seperti chlorpromazine dan thioridazine cenderung menunjukkan tingkat terbesar dari efek antikolinergik di kelas, sedangkan haloperidol serta yang lebih baru, atipikal agen seperti quetiapine, risperidone dan ziprasidone umumnya telah dikaitkan dengan frekuensi sangat rendah antikolinergik efek samping. Terapi dengan neuroleptik agen harus diberikan hati-hati pada pasien dengan kondisi yang sudah ada sebelumnya yang mungkin diperburuk oleh aktivitas antikolinergik, seperti retensi urin atau obstruksi; glaukoma sudut tertutup, tidak diobati hipertensi intraokular, atau tidak terkendali primer glaukoma sudut terbuka; dan pencernaan gangguan obstruktif.
