Aminoglikosida dapat menyebabkan saraf kranial kedelapan kerusakan, sehingga vestibular dan/atau toksisitas auditori. Gejala-gejala seperti pusing, nistagmus, vertigo, ataksia, tinnitus, dan berbagai derajat gangguan pendengaran. Kehilangan pendengaran permanen dapat terjadi, termasuk, jarang, total atau parsial ireversibel bilateral tuli setelah obat telah dihentikan. Terapi dengan aminoglikosida, terutama jika berkepanjangan (> 10 hari), harus diberikan hati-hati pada pasien dengan preexisting vestibular dan/atau gangguan pendengaran, karena dapat menunda pengakuan atau mengacaukan diagnosis akibat obat ototoksik efek. Serial audiograms harus diperoleh pada pasien yang cukup tua untuk diuji, karena kehilangan tinggi - frekuensi persepsi biasanya mendahului klinis gangguan pendengaran. Dosis harus dikurangi atau terapi ditarik segera jika tanda-tanda dan gejala toksisitas berkembang.
